Provides entry patient care coordinator (PCC) skills for evaluating, planning, and advocating for patient health needs. Trains students to monitor and coordinate patient treatment plans, to understand health status, and check progress in an ambulatory. Part one of a two-part series.

To succeed in today’s emerging healthcare models such as health homes, patient centered medical homes and accountable care organizations, frontline healthcare staff members are being asked to serve as abridge between patients and providers. To accomplish this, frontline staff members require more advanced skills and training than they have received in the past. Specifically, they will need patient navigation and care coordination skills.
Following the evaluation of current levels and types of career education programs through the Regional Strong Workforce Program (SWP), the Regional Consortium found a need to develop a Patient Care Coordinator (PCC credit course) and PCC VESL (non-credit) course. It was determined that enhanced noncredit adult education coursework and programs funded through the Regional Strong Workforce Program (SWP) would be developed to improve enrollment and completion in these programs.

Southwestern College submitted the appropriate LMI from the COE. The COE recommends proceeding with developing a program because 1) a supply gap exists for these occupations; 2) their entry-level and most median earnings are below the living wage; and 3) a high number of annual openings exist. These occupations support a career pathway for Patient Care Coordinators. Patient Care Coordinators ensure that health care facilities (e.g., hospitals, physicians’ offices, clinics, long-term care facilities) provide high-quality services to their patients.
